October ASC update changes codes, skin substitutes and retroactive pricing

The October ASC update mixes new codes, revised descriptors and retroactive payment changes. Its effective dates vary by item, and two specified drug-code groups require contractor reprocessing. A code and payment rate still do not establish coverage; providers must apply the relevant code tables and service dates.

What happened

CMS transmittal 13983, CR 14634, describes the October 2026 ambulatory surgical center payment update, generally effective October 1 with October 5 implementation. The October 8 document includes ten new category III codes, replacement of C8010 by 1091T, twelve new drug pass-through codes and eight new skin-substitute product codes. Five skin-substitute descriptors change retroactively to April 1. Stimuplex needles qualify for the specified additional non-opioid payment under C9812 beginning April 30. These different dates require item-specific validation rather than one uniform file cutover.

The instruction also requires contractors to search and reprocess appropriate A9521 claims for May 14–September 30 service dates and J9161 claims for July 1–September 30, within thirty days of implementation. Other restated drug prices may be addressed through provider resubmission, so that general option should not replace the two mandatory contractor searches. CMS expressly cautions that an assigned code and rate indicate how a covered service may be paid, not whether it is covered. Claim review therefore needs the applicable descriptor, payment indicator, service date, coverage rules and supporting clinical documentation.

Why it matters

The update replaces C8010 with 1091T; revises skin-substitute add-on descriptors retroactive to April 1; adds codes and retires drug codes September 30. MACs reprocess A9521 from May 14 and J9161 from July 1 through September 30 within 30 days of implementation. Stimuplex joins C9812 effective April 30. A code or rate does not establish coverage.

Implementation playbook

  1. Map the affected claims and operative dates with the accountable owner. Coding · Due 2026-10-18
  2. Load the applicable code, descriptor and payment-indicator tables; test date-specific claim edits and reconcile affected reprocessing against actual coverage and documentation. IT · Due 2026-10-25
  3. Reconcile representative claims and retain implementation evidence before the operative release. Revenue Cycle · Due 2026-11-01
