Recuperative care changes preserve phased housing sunset and member transitions
Oct 11, 2026 edition · GC-2026-0323 · Published 2026-10-11 · Version 3 · Final
Final recuperative-care standards begin in January, while the housing sunset requires earlier notices and authorization controls. The December cutoff has a specific UIS-transition exception.

What happened
The revised definition takes effect January 1, 2027; the current definition continues through December. Medi-Cal stops covering room and board, although facilities must still provide beds and meals. Required care includes onsite coordination, daily wellness checks, safe medication storage, and qualified supervision. All supervisors need staff or team-management experience and regular onsite availability; LCSW and MSW pathways also require specified health-care experience. The six-month rolling service limit continues. NIMRC certification is encouraged, and DHCS does not impose a new January facility-licensure requirement.
New STPHH authorizations stop after December 1, except that the cutoff is December 31 for members transitioning from managed care to FFS under the UIS transition. No authorization may extend beyond December 31. Plans must issue standardized member notices by December 1 and support connections to appropriate services, rather than assume every member qualifies for recuperative care. Shortening a requested authorization can trigger appeal rights. Earlier room-and-board months remain relevant to the rolling global-cap runout even when later recuperative-care months no longer count.

Why it matters
MCPs offering STPHH must send standardized member notices by December 1, support transitions, update provider agreements and January materials, and report monthly through December; the last report is due January 10. Reduced requested authorizations can trigger appeal rights. Earlier room-and-board months still count during the rolling-cap runout.
Recommended action
Case Management and Managed Care should identify every affected member and prepare notice, referral and contract transitions. For January recuperative care, check supervision, daily wellness checks, safe medication storage and six-month rolling limits. NIMRC certification remains encouraged, rather than newly mandatory.
