SNF FY 2027 correction revises wage-index calculations and burden estimates

CMS corrects SNF wage-index calculations and reporting-burden estimates through separate technical adjustments. The correction changes rate-setting inputs and tables while leaving the stated payment methodology and displayed aggregate impact analysis intact.

What happened

The September 30 correction restores hospital wage data wrongly excluded when an IPPS hospital was classified as a rural emergency hospital. That restoration requires recalculation of the SNF wage-index budget-neutrality factor, unadjusted federal per diem rates, case-mix-adjusted rates, and related PDPM and wage-adjustment examples. CMS says the impact analysis in Table 25 is unaffected at the displayed precision. The correction is effective October 1 as part of the final-rule framework; its financial effect still depends on the applicable corrected tables and facility wage adjustment.

The separate SNF Quality Reporting Program error concerns use of mean rather than median Bureau of Labor Statistics wages in burden calculations. Correcting those wage assumptions changes reported burden estimates, rather than establishing that every facility experienced that amount of actual administrative cost. CMS characterizes the document as correcting technical and typographical errors without substantive changes to adopted policy or payment methodology. Rate-table reconciliation and review of the corrected burden discussion are therefore distinct tasks, and neither supports applying one new universal percentage to every SNF claim.
