HHS asks whether the federal vaccine recommendation categories should change, comments close September 20

The September 20 comment window on federal vaccine-recommendation categories is closed; the RFI did not itself adopt new categories or change clinical recommendations.

What happened

HHS's August 24, 2026 request for information asked whether federal vaccine recommendations should continue to use routine, risk-based and shared clinical decision-making categories, or adopt different categories. Comments were due September 20, so this October 4 record remains closed as a comment opportunity. The notice sought input to inform the department and its vaccine task force; it did not itself adopt a new recommendation structure. Its description of existing categories distinguishes routine and risk-based recommendations, where vaccination is generally the default absent contraindications, from individualized shared decision-making, where the patient's circumstances and preferences shape the decision.

The RFI also asked how scientific evidence, limited randomized-trial evidence, individual autonomy, religious freedom and public communication should influence the framework. It expressly raised downstream legal and program consequences of category assignment. That makes the record relevant to organizations whose order sets, education materials, forecasting tools or coverage explanations refer to a recommendation category, but the notice alone is not a reason to rewrite those materials as if a new category had taken effect. Preserving a record of category-dependent documents can support later review if an authoritative recommendation changes. The expired request should remain separate from monitoring for any later agency action.

Why it matters

A category change could affect clinical communications and program workflows, but this closed RFI establishes an information-gathering event rather than a new clinical or coverage rule.
