AB 1199 changes medical staff reappointment and competency review intervals
Oct 11, 2026 edition · GC-2026-0060 · Published 2026-10-11 · Version 3 · Final
AB 1199 moves specified California medical-staff reappointment and recurring competency requirements to a three-year interval. The September 30 enactment also bars CDPH from requiring more frequent routine reappointments at general acute care and acute psychiatric hospitals, while retaining medical-staff oversight requirements.

What happened
California chaptered AB 1199 on September 30 as Chapter 876. It changes Business and Professions Code section 2282 so covered hospitals' medical-staff rules provide for reappointments at least every three years, replacing the annual-or-biennial language identified in the legislative digest. The related physician-staff competency provision in section 2453 requires a satisfactory demonstration of the ability to perform surgical and other procedures at the original appointment and at least every three years afterward, rather than the prior two-year interval.
For general acute care and acute psychiatric hospitals, amended Health and Safety Code section 1275.6(e) places the competency-control responsibility on the governing body and medical staff, with demonstration before an appropriate medical-staff committee at original appointment and every three years thereafter. The same provision prohibits CDPH from requiring routine reappointments more frequently than every three years. The surrounding provisions continue to require competent, ethically qualified staff and periodic review of clinical experience using patient records. The interval change therefore leaves those continuing oversight responsibilities in place.

Why it matters
Hospitals and medical staffs may need to revise credentialing calendars, bylaws, and reappointment controls from a two-year to a three-year cycle.
Recommended action
Medical staff and Legal should map the final interval and exceptions to bylaws, privileging systems and other applicable requirements before changing renewal cycles.
