AB 1682 adds qualifying scalp cooling coverage

AB 1682 separates large-group scalp-cooling coverage from a conditional Medi-Cal benefit. FDA clearance, prescribed chemotherapy-related use, and cost-sharing limits define the commercial requirement.

What happened

Large-group contracts or policies issued, amended, or renewed from January 1, 2027 must cover prescribed FDA-cleared scalp cooling around alopecia-inducing chemotherapy. Cost sharing may apply, but cannot be less favorable than oncology supportive care. Non-FDA-cleared cold caps are excluded, as are specialized commercial coverage and Medi-Cal managed-care contracts under the commercial provision.

Medi-Cal has a separate prescribed scalp-cooling benefit subject to medical necessity and utilization controls. Its definition likewise excludes non-FDA-cleared products. Implementation depends on necessary federal approvals and available, unjeopardized federal participation; DHCS may issue administrative instructions. These conditions preclude assuming immediate universal reimbursement or applying the large-group mandate to every individual or small-group policy.
