SB 950 adds qualifying dementia treatment coverage protections

SB 950 combines qualifying dementia-treatment coverage with limits on step therapy and authorization criteria. Its antiamyloid alternative, medical-necessity review, benefit scope, and excluded contracts prevent a blanket rule for every product or payer.

What happened

Contracts or policies issued, amended, or renewed from January 1, 2027 must cover medically necessary FDA-approved treatments for Alzheimer's disease or other memory conditions, as determined by a health care provider. The provision reaches medical benefits and prescription-drug benefits to the extent the contract provides them. Its step-therapy restriction addresses both self-administered and physician-administered drugs, but does not require all treatment types to be available without step therapy when at least one antiamyloid therapy is covered without it.

Other utilization management, including prior authorization, remains available when medical necessity and appropriateness are assessed in the same manner as for other covered conditions. Criteria cannot be more restrictive than FDA-approved indications, and authorization for medically necessary covered treatment is treated as an exigent circumstance. The plan provision excludes Medi-Cal managed care, dental-only or vision-only specialized contracts, and Medicare supplements; the insurance section also lists specified limited-benefit exclusions. These conditions call for product- and contract-specific configuration rather than removing every dementia utilization edit.
